Helpful treatments keep people with arthritis

But what if joint pain and stiffness are inevitable if you don't take the medication, yet heart problems could occur if you do? Health officials say that, as with any drug, only you and your doctor can determine the level of risk that is acceptable with medications currently available to treat arthritis.


The unsettling news in late 2004 that the popular anti-inflammatory arthritis drugs Vioxx (rofecoxib), Celebrex (celecoxib), and Bextra (valdecoxib) could cause a heart attack or stroke or aggravate high blood pressure has left some patients wondering whether they should keep taking them.


Data from clinical trials showed that cyclooxygenase-2 selective agents, better known as COX-2 inhibitors, may be associated with an increased risk of serious cardiovascular problems, especially when used in high doses or for long periods in patients with existing cardiovascular disease, or in very high-risk situations, such as immediately after heart surgery. COX-2 inhibitors are the newest subset of non-steroidal anti-inflammatory drugs (NSAIDs). COX-2 inhibitors were developed specifically to decrease the well-recognized gastric side effects and intolerance associated with the use of some NSAIDs.


Traditional NSAIDs, such as aspirin or ibuprofen, act by blocking the production of a family of chemicals known as prostaglandins, which are not only important in the development of inflammation, but also play an important role in maintaining the integrity of the stomach lining. At least two enzymes are involved in this inflammation, namely cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2). Traditional NSAIDs inhibit both COX-1 and COX-2. Unfortunately, this non-selective inhibition of both COX enzymes also inhibits those prostaglandins involved in some of the important "housekeeping" functions of the body, such as helping blood to clot and protecting the stomach from ulceration.


It is this non-selective inhibition of both enzymes by aspirin and other non-selective NSAIDs that increases the risk of stomach ulcers and consequent bleeding. In theory, the newer COX-2 selective inhibitors only inhibit the enzyme involved in inflammation, leaving the other functions alone.


But Sandra Kweder, M. D., deputy director of the Food and Drug Administration's Office of New Drugs, says that new studies have had a surprising twist. "The downside of the COX-2 selective inhibitors is that they appear to be associated--particularly when used for many consecutive months to years--with an increased risk of cardiovascular problems," she says. Moreover, COX-2 inhibitors, like other NSAIDs, don't influence the course of the disease--which doctors say is a common misconception among patients--because these drugs only provide for symptom relief.


Coincidentally, preliminary results from a three-year trial on the non-selective NSAID Aleve (naproxen) also suggested that long-term use may be associated with an increased risk for cardiovascular problems.


Since the results of studies on both non-selective and selective NSAIDs are preliminary and conflict with data from earlier studies of the same drugs, the FDA issued a public health advisory in December 2004 concerning use of all NSAIDs, pending further review of data that continue to be collected. The agency has recommended, among other things, that physicians limit the use of COX-2 inhibitors until further review.


"Monitoring the drug safety of marketed products requires close collaboration between our clinical reviewers and drug safety staff to evaluate and respond to adverse events identified in ongoing clinical trials or reported to us by physicians and their patients," says Kweder. "The most recent actions concerning [NSAIDs] illustrate the vital importance of the ongoing assessment of the safety of a product once it is in widespread use."


Others Weigh In


The concerns with the safety of so many pain relievers used to treat arthritis underscores the importance of arthritis as a major health care issue in the United States. Arthritis experts, patient advocacy groups, and other health organizations also are weighing in on the uncertainty of NSAIDs, and are recognizing the need for developing new and safer medications.


The American College of Rheumatology is advising physicians to follow current treatment guidelines and manufacturers' dosage recommendations for NSAIDs. Treatment guidelines exist to help doctors choose the best options for their patients, based on current scientific studies.


The Arthritis Foundation said in a statement that the findings about the drugs should not have any immediate impact on the clinical use of them.


"Non-steroidal anti-inflammatory drugs continue to play an important role in the management of arthritis pain," says John H. Klippel, M. D., president and CEO of the Arthritis Foundation. "Patients who derive benefit from these drugs should remain on their treatment regimen, and discuss concerns with their physicians," he says.


But Charles A. Birbara, M. D., a rheumatologist and associate professor of medicine at the University of Massachusetts Medical School in Worcester, Mass., says that he has prescribed COX inhibitors cautiously in older people or those with cardiovascular or renal disease ever since early clinical studies discovered a possible risk in this patient population.


"I'm not always willing to take a risk with my patients, because we clearly don't have a complete understanding of all the potential clinical issues associated with use of these drugs," he says. Even before the controversy, Birbara notes that the long-term effects of COX-2 agents were unknown. Besides, he says, there are other treatment options available that are equally effective.


"There are so many wonderful things happening with respect to current therapies of arthritis," he says. "Clearly, we are so much better able to control inflammatory arthritis with new biologic agents." Birbara adds that so many people whose lives were diminished by joint disease can now look forward to an unrestricted lifestyle, which was "not even imaginable just a few short years ago."


Richard Shirley, 64, an avid birdwatcher from Wrentham, Mass., battled rheumatoid arthritis in nearly every joint in his body for more than 25 years before he saw Birbara. He couldn't button his shirt cuffs, walk frontward down a flight of stairs, or get in and out of a car without a struggle. At times, his hands were so swollen he couldn't grasp small objects or make a closed fist. Shirley had his wedding ring resized so it would fit.


"Richard had seen a number of physicians and had been on many medications to treat his disease," Birbara says. "However, the aggressive nature of his arthritis was not very responsive to standard medications." According to Birbara, X-rays of Shirley also showed evidence of joint destruction.


"Doctor Birbara has a zero tolerance for hot and inflamed joints," Shirley says, "because that's when the damage is done." Shirley also believes, from his own experience, that each person needs to work with his or her physician to find the appropriate medicine. For him, a new biologic product made the difference.


Finding the right treatments for those at greatest risk for the potential complications of arthritis and other rheumatic conditions, Birbara says, hopefully will lessen the burden of this disease, not only in the United States, but for the entire world.

Trying to understand adhd

ADHD is Attention Deficit Hyperactivity Disorder. It is a mental disorder that approximately three to seven percent of children have. Those that are affected by this disorder are characterized by constant behavior, lots of activity and often thought of as being disobedient. Yet, it is not that the individual is being bad but more so that they cannot control their mental range. Instead of being able to concentrate on one element, they are likely to be thinking about several thus making it quite difficult to concentrate.


This disorder is not just found in children. Many adults are diagnosed with it as well. In adults, it is classified as AADD, or Adult Attention Deficit Disorder. About 30 to 70 percent of all children diagnosed with ADHD will carry on with their disorder through adulthood. It is harder to detect in adults because they have learned to adequately handle the disorder. By learning to live with it and work around it, adults have less of a need for help. Yet, in many cases both children and adults will need medications to help focus attention.


Symptoms of this disorder in children will be such things as inattentiveness, impulsive behavior and constant restlessness. These children have a hard time sitting still or concentrating on just one thing for a very long period of time. In adults, it is more difficult to diagnose. It can be seen in the inability to structure their lies and to plan their daily activities. It is often less of a problem for them to stay attentive and to stop restlessness simply because these are secondary problems to daily planning of tasks.


ADHD is a disorder that can be treated but not cured. It can be lived with or medication may be necessary. Seeking the assistance of medical personnel is necessary in any case.

What are the most common asthma triggers

An asthma attack is often triggered by exposure to an allergen of some sort – an asthma trigger. Identifying a food or environmental asthma trigger can help you avoid it – and lessen the risk of an acute asthma attack. While there are some people who have food allergies or are especially sensitive to particular things, there are some items that are well-known, common asthma triggers. By controlling your exposure to an asthma trigger, you can reduce the severity and frequency of your asthma symptoms.


Dust Mites


Dust mites are one of the most common asthma triggers known. Dust mites are tiny insects that live on sloughed off skin cells and excrete a substance that tends to trigger attacks in many people who have asthma. Dust mites breed and grow in mattresses, pillows, stuffed animals, carpets, draperies – anywhere that is soft and offers somewhere for them to burrow and hide. You can control dust mites in your environment by using covers on mattresses and pillows, getting rid of stuffed animals in bedrooms, and remove carpets and draperies.


Dust


Separate from dust mites, dust itself can be an asthma trigger because it’s a bronchial irritant. Wipe down surfaces with a damp cloth to keep from scattering dust when cleaning, and use a canister (or water-filtered) vacuum cleaner to avoid spewing dust from the carpet into the air to be inhaled.


Mold


Mold spores are another asthma trigger found in many homes. To keep the exposure to mold down, wipe down bathroom tiles regularly with bleach or a disinfectant, dry clean laundry immediately and reduce moisture in the air with a dehumidifier.


Cockroach Parts


Cockroach litter and parts contain an irritant/allergen that is an asthma trigger for many children. While most people associate cockroaches with dirt and poor housekeeping, cockroaches are just as happy in very clean homes. Roach traps and other insecticides can help keep the cockroach population down.


Pet Dander


Shed skin cells from pets can be one of the most virulent of asthma triggers. If you can’t bring yourself to part with your pet, at least keep it out of the family sleeping areas to reduce the chance of exposure to pet dander.


Pollen


At certain times of the year, pollen can become a pervasive asthma trigger. During hay fever season, particular if you live in an area that also has poor air quality. Many newspapers and radio stations publish a daily air quality rating. Make a point of knowing when the air quality is unhealthy and avoid going outside, or at least engaging in any vigorous activity outdoors.


Cold Air


Cold dry air can trigger an asthma attack in people who are sensitive to it. It may be because the cold tends to dry out bronchial tissues more quickly than warm moist air. In any case, wearing a scarf or face mask in cold temperatures, particularly if you’re exercising at all, can help reduce the incidence of asthma attacks due to cold.


Exercise


Exercise can sometimes be an asthma trigger. The reason, doctors think, is most likely that during exercise most people breathe more quickly and shallowly, drying out lung and bronchial tissues more quickly.


Food Allergies


Food and other allergies can also trigger asthma attacks. These are far more individualized, though peanuts are one of the more common. Obviously, if you’re aware of food allergies, avoiding them can help reduce your risk of having an asthma attack.

Emotional stress and crohn s disease - how they relate

For years, many people have argued that forms of Inflammatory Bowel Disease, including Crohn’s Disease, is caused by emotional stress, tension, and anxiety; this, however, is completely inaccurate. While there may be links to emotional stress and Crohn’s Disease, it is not a cause of the disorder.


A common misconception is that Inflammatory Bowel Disease (IBD) is the same thing as Irritable Bowel Syndrome (IBS). However, these differ considerably. IBD causes inflammation of the intestines and is not caused psychological factors. On the other hand, there is a strong link in emotional issues contributing to IBS.


While some may argue Crohn’s Disease is caused by being overly emotional, it is in fact an inappropriate response of the immune system and no correlation to the disease and psychological issues. However, emotions may play a role in how a person deals with Crohn’s Disease.


In dealing with any chronic disease, people may find difficulty is coping with the long term effects of their medical condition. As a result, people with Crohn’s may become depressed or suffer other psychological issues, such as severe anxiety or dependency. As Crohn’s often causes excessive diarrhea and gas, it is sometimes embarrassing for people to go out in public, possibly leading to reclusion. Likewise, travel becomes more difficult, which may lead to feelings of loss of freedom. Long term pain also causes emotional complications, as well as long term use of pain medications.


While Crohn’s Disease may cause emotional stress, there has also been a reverse connection recognized: meaning stress may cause flare-ups of complications in patients already affected by Crohn’s Disease. Through extremely emotional times, a person may notice an increase in severity of diarrhea, gas, and pain.


If psychological issues become a hindrance in daily life, it may be necessary to seek professional help. While medication is not typically needed, simply acquiring support may be very beneficial. This may also be accomplished by joining support groups or conversing with other people that suffer from the same or similar disorders.


For peace of mind, there are also other steps that you can take to reduce stress as a result of Crohn’s Disease. Carrying a change of clothes with you, familiarizing yourself with your surroundings, including location of bathrooms, and being aware of your body’s reaction to certain foods will ease anxiety when you are out and about and prepare you for possible issues that may arise.

Host foreign species - infection

An infection is the in short the hosting of foreign species in a host organism. There is a war that is involved between the host and the foreign body as the infection organism is looking to use the hosts resources in order to multiply. All of this occurs at the expense of the host. If the host and the intruder could coexist there would be no problem, but this isn't the case. The infecting organism in it's intrusion will interfer with the normal operation of the host, and in many cases inflict with the survival of the host. These organisms are usually consired to be tiny and microscopic, however the definition is in fact much wider than that.


Pathogens are a broad group that include: (Infectious Disease is the specific branch of medicine that focuses on these infections and pathogens.)


- Parasites


- Bacteria


- Viruses


- Prions


- Viroids


- Fungi


All organisms with multiple cells(multicellular) are colonized to some extent by an extrinsic organisms. The large majority of these exist in either a relationship with the host like mentioned above. The difference between an infection and a colonization is due to the circumstance which exists as a result of the former. Basically if the colonization of the external body comprises the life of the host organisms it is an infection.


Several variables are involved which determine the intruders outcome on the host:


- The Route of entry


- The access to host regions that it gains


- The amount or load of the initial inoculant


- The intrinsic virulence of the particular organism


- The status of the immune system of the host that is being colonized
